When the worm is directly under Mario, press down on the D-pad to do a ground pound. Repeat 3 times to defeat the worm. At the end of the level, Mini-Mario will be able to fall through the gap in the floor that leads to World 4. If you are having a hard time getting to the end of the level as Mini-Mario, revisit World 1-4.
